- The sessions in your plan can be a combination of both psychotherapy
and substance abuse.

- This is a program, paid for by the company, that proactively provides
their employees’ families assistance to live a better quality of life.
- The program includes both free psychotherapy sessions and substance
abuse programs.

- The sessions provided for in the EAP is not only for each employee, but
the total number of sessions may also be used by immediate employee
family members.

During 1 year
- Joe has 2 sessions

- Total of 6 sessions = Free for family (Depending on EAP plan chosen)

- No matter what type of referral is made, all information is kept
confidential to the full extent of the law.
- Self-Referral: Employee calls and
sets up an appointment for employee or a family member.
- Totally confidential access to help.

- Suggested Referral: Employer
refers employee to Counseling Associates, but it is up to the employee
to go.
- This might happen if a supervisor notices that something is affecting
the job performance of an employee and attempts to be proactive.
- The employer will never know if the employee goes or not unless the
employee signs a release.

- Mandated Referral: A condition of
continued employment is obtaining counseling at Counseling Associates.
- Employer calls before employee and informs CA of referral.
- When employee calls, CA calls the employer to simply let them know the
appointment has been made.
- Everything done in session is confidential unless the employee
voluntarily signs a waiver.

- During the first 3 sessions of psychotherapy a multi-axial, DSM=IV
diagnosis of the person(s) involved will be done, as appropriate.
- CA will offer referrals to what we believe are the providers of the
best and most appropriate care for the situation.
- We will give the client as much information on his/her treatment
options, so the client can make an informed decision on his/her
treatment path and the costs involved.

- This does not provide for substance testing.
- This is for the employee’s entire family.
- CA provides instruction for supervisors on how to identify and refer
employees to CA.
- CA provides up to one on-site counseling help in case of company
emergency.
